At Beloved Wellness Center®, we currently accept one health insurance plan: Network Health. This decision was made with care and intention. Factors such as reimbursement rates, administrative efficiency, and the ability to maintain high-quality service influenced our choice.
If you have private insurance, you may also choose to opt out of using your plan and pay out of pocket for greater privacy and flexibility. Please note that Beloved Wellness Center® does not communicate or intervene with insurance companies on your behalf.

Privacy: Your confidentiality matters deeply. When insurance is used, we may be required to share clinical documentation to justify continued care. This limits full confidentiality. Additional details will be reviewed during your initial assessment and outlined in your consent forms.
Control: Insurance companies decide which services are covered, how often you can attend, and how much they will reimburse. Paying privately gives you more control—you can choose the type of support you need, how frequently you meet, and the provider you trust most.
Specialization: Dr. Dominique specializes in working with adolescents, adults, and families through a culturally responsive lens. Insurance networks often limit access to generalist providers only. Paying privately allows you to work directly with a specialist who aligns with your needs.
Wait Times: Because many Wisconsin residents are required to carry insurance, most in-network providers maintain lengthy waitlists. Beloved Wellness Center® does not keep a waitlist. Once you submit your pre-screen form, you can expect a response within 24–72 business hours. Approved clients typically begin services within a week.
Frequency of Sessions: Even the best insurance plans often cap the number or frequency of sessions covered. In some cases, meeting more than once a week can support faster, lasting progress. Private payment allows flexibility based on your needs and availability—not insurance limitations.
Diagnosis: Insurance billing requires a formal mental health diagnosis. While some find this useful, it can also create unnecessary labels early in care. At Beloved Wellness Center®, we believe in focusing on your experience and goals first—not a diagnosis to justify your need for therapy.

All clients are required to keep a credit card on file for business purposes, which will be explained in your intake paperwork. Your card information is securely stored and will never be misused.
Once services end and your account is in good standing, your card will be deleted from our system.
If you pay out of pocket and your card is declined, you will have 48 hours to update your payment information before your file is closed or future appointments get cancelled.

We serve Black women, teen girls and active allies (those advocating on behalf of historically disenfranchised groups) ages 15 to 50. If you are seeking family therapy, the identified client must be within this age range.
We provide individual, family (parent/guardian/child - adult or minor), friendship and speciality group therapy. We do not provide couples counseling.
You must be at least 18 years old to register for services and complete the prescreen form. If you are under 18, do not continue. A parent or legal guardian must complete this form on your behalf.
No, we do not offer (free or paid) phone consultations. Our decision to offer a first appointment is based on what you put on this form. We will focus on your presenting concerns, what you hope to gain from therapy, your goals, possible next steps and whether we are a good match to work together.
Unfortunately, we do not keep a waiting list.
Yes. If your pre-screen is approved, you will receive a link to request a new client appointment.
After you submit the request, it will remain pending until all intake paperwork is completed, returned, reviewed for accuracy and your payment clears if you are paying out of pocket.
We use this paperwork to determine whether you are a good match for our agency.
Check your email inbox or spam folder for a message from Beloved Wellness Center or Client Portal with the paperwork link.
You will have 72 hours from the time the initial paperwork is sent to complete ALL doucments or your file will be closed.
If you are self-pay or private pay, you will need to pay the full intake session rate of $200. If you have been approved for a reduced rate, you will pay the agreed-upon amount.
Your appointment will be approved once the payment clears, which may take anywhere from a few minutes to a few hours.
If your payment does not clear, you will be notified by email and have 24 hours to update your payment method. If payment is not received within that time, your file will be closed.
If you are using insurance, no payment is required before your appointment.
On the date of your session, you may be responsible for a co-payment based on your provider’s contract. This amount will be shared with you in advance and you will be asked to sign an acknowledgment confirming your understanding.
